Did you receive notification that your application or a particular payment was denied?
The reason for denial should be listed on the notification letter or email you received. If you think this may have been an error, you have an opportunity to appeal the decision. Here is how:
Submit a Request for Reconsideration by mail, fax or secure email within 30 days of receiving the decision letter.
If you have additional documents you would like included in the decision process, please submit them with your Request for Reconsideration.
If you have a reasonable explanation for why you may have missed a deadline, such as a hospital stay or a minor child who did not immediately disclose, please explain this circumstance on your Request for Reconsideration. You can verify eligibility criteria here.
